In order to be considered for degree candidacy, each student MUST FILE AN APPLICATION with the Registrar's Office in accordance with the following schedule:

October | the preceding D-Term |
Complete the Application for Bachelor of Science Degree as soon as possible. If you revise your date of graduation, or do not complete your program as planned, you MUST submit this form for future consideration.

The application process is to be completed during the semester in which the student anticipates completion of all degree requirements. Graduate student must be registered in the semester in which the degree requirements are completed. Return the form below to:

For October Graduation; Application Submission prior to July 1
For February Graduation; Application Submission prior to October 1
For May Graduation; Application Submission prior to December 1
