Application for Bachelor of Science Degree
In order to be considered for degree candidacy, each student MUST FILE AN APPLICATION with the Registrar's Office in accordance with the following schedule:

February | the preceeding A-Term |
May | the preceeding B-Term |
October | the preceeding D-Term |
Complete the Application for Bachelor of Science Degree as soon as possible. If you revise your date of graduation, or do not complete your program as planned, you MUST submit this form for future consideration.

The application process is to be completed during the semester in which the student anticipates completion of all degree requirements. Graduate student must be registered in the semester in which the degree requirements are completed. Return the form below to:

For October Graduation 2007; Application Submission by July 2007
For February Graduation 2008; Application Submission by October 2007
