• Completion of Degree Requirement (CDR) Forms:
  • CDR - Sufficiency (PDF form which may be filled in electronically)
  • CDR - Humanities and Arts Requirement (PDF - with fillable fields)

Forms are also available from the Humanities and Arts Department or the Projects and Registrar's Office.
