Summer Programs Required Forms
At WPI we work hard to provide an environment that supports the health and well-being of all members of the WPI community. Our Health center provides multifaceted professional medical care. In order to take the best possible care of each student, you must fill out the required forms.
Please print each form and fill it out, then e-mail them to: summer@wpi.edu or mail them to: Summer Programs, 100 Institute Road, Worcester, MA 01609.
Please submit your forms through the online registration system. If you did not use the online system to submit your application/registration or if you have trouble accessing the system, please call the summer programs office at 508-831-4900.
