2012 Workshop Request to Participate Form

Each attendee must fill out and submit this Request to Participate form by or before June 1, 2012.

Please Note: Attendees will be limited to those accepted by the 2012 Workshop Committee to participate in the workshop.

Notification of acceptance will be made by June 22nd, 2012. If accepted, the registration deadline will be July 13th, 2012.

There is no registration fee required. All accepted attendees are responsible for thier own travel costs to and from the workshop. (Members of the first responder community may request a a travel grant of up to $500.00)
