We are excited to welcome you to the 2012 WPI Launch program! We have created this site to share important information with you. In advance of your arrival, please complete the following forms, and submit them via email to launch@wpi.edu or via fax to 508-831-5875.
- Office of Summer Programs Participant Agreement Form
- Office of Summer Programs Medical Waiver Form (sss)
- Physical and Immunization Form (You may use this form or one provided by your physician)
- Launch Voluntary Disclosure Form (optional)
