  Use this form to apply for any non-student project (i.e. not an IQP, MQP ) exemption from IRB review for survey or interview research involving minimal or no risk. Generally an exemption is available for work involving educational practices in educational settings, and survey or interview work that involves no risk to subjects if information is divulged.
