Patient data is fragmented, incomplete, and inaccessible between service providers, patients, andpayers. In addition to data interoperability challenges, providers also have no visibility of theservices patients are receiving from multiple providers. This inability to share knowledge on bothdata and patient services, inhibits care coordination, leads to lower health outcomes, higher patientrisks and sky rocketing costs. This conference brings together industry leaders to discuss:
