Non-Payment Forms
Forms that do not require payment, can be created in Qualtrics. Qualtrics is easy to use, gives you full control over editing your forms, and direct access to form submission data. Here are some links to get you started:
- Log into Qualtrics at https://wpi.qualtrics.com/login (If this is your first time using Qualtrics, select the “I do not have an account” option to begin.)

Request a Payment Form
If your form requires payment by Credit Card, please complete this Qualtrics survey. When you submit this form, a ticket will automatically be sent to us and we will contact you with any questions or to let you know when the form is ready for your review.
- Note that we need 2 weeks lead time to create and test a form, so please plan accordingly.
- Before completing this form, please contact Michelle Pickett to request Workday tags for this form. We will not be able to create the form without them.
